Patients complete the Patient Demographic Information and Authorization for Release and Use of Confidential Patient Information forms (see below) at their first appointment and bring a copy of their current medications. Or, you are invited to print and complete the forms below and bring them to your appointment.

While HOC is “in network” for many insurance companies, you need to verify that our practice is in network for your particular plan. If HOC is “out of network,” you will need to pay additional charges as determined by your insurance company.

Your health insurance card(s) will be copied in the office. If your insurance requires a referral, this should also brought to your first appointment.

Patients referred to HOC from another physician (primary care, surgeon or specialist) should request the referring office send the appropriate records prior to your visit via fax to the office of your scheduled appointment: Naperville’s fax: 630.369.1560; Darien’s fax: 630.985.1969.

If you are coming to HOC from another hematology/oncology practice, you may need to submit a HIPAA Authorization & Release of Information form to authorize the transmission of your medical history and test results to our office.
