THRIVE Cancer Survivorship Program

Our survivorship program is available for all of our patients completing chemotherapy, and is led by physician assistant, Carolyn O’Connor, PA-C. One-on-one meetings usually last for about one hour, and are comprised of three important components of survivorship. Firstly, we discuss your personalized Treatment Plan and Summary, which is a comprehensive document summarizing your diagnosis, surgery, chemotherapy and radiation treatment, etc.

The second part of the meeting addresses concerns of survivors, including physical, psychosocial, and emotional matters. And finally, we develop a personalized Care Plan, which details your best schedule for follow-up exams and testing, and addresses any further concerns. Copies of documents are then sent to you, as well as your other physicians, to better coordinate your care across your healthcare team.
